The Application Form and Record Sheet for the POLSKA Award are provided through an interactive online form that enables applicants to complete the required fields electronically and generate PDF documents. The generated PDF files can be opened, reviewed, and printed using PDF readers such as Adobe Acrobat Reader or Foxit Reader.
